This form is used by Team Nic volunteers to enter follow-up actions from door knocking sessions.

It prompts for the householder's details, which they provided during the conversation with our door knocking team. 

Entering data on this form will create a record in NationBuilder (our CRM) for the householder and (depending on the data): 

1. Subscribe them to the newsletter
2. Assign them to the Volunteer path as a potential volunteer
3. Identify them as wanting a corflute closer to the election
4. Log their specific question
